Diabetes & Medication Monitoring
DIABETES PROGRAM: Aims to educate diabetic clients about the disease and how to control it. RGH Pharmacy Consulting Service P/L provides a pharmacist to Pika Way; includes home medicines reviews in collaboration with GPs to improve medication compliance, identify side effects of some medications, the cessation of unnecessary medication and regular reviews of medication.
Health Education and Self-Management
HEALTHY FOR LIFE PROGRAM: early intervention, prevention and management. Includes the DIABETES PROGRAM: Aims to educate diabetic clients about the disease and how to self manage; 1:1 and community education. Visiting Respiratory, Physiotherapist, Occupational Therapist and Respiratory Nurse. Endocrinologist visits once a month.
Nursing - Clinic Based & In Home
Health care workers provide general nursing. Child and Youth Health Nurse available every Tues and Fri half day; service includes general health checks. Home visits can be arranged. Traditional healer available on a needs basis.
General nursing care through the Aboriginal Health Care worker i.e. bloods pressure, blood sugar levels, dressings, interpreting and others.
Transport - Personal
TRANSPORT SERVICE: provides transport to and from the clinic to those clients in most need. PATIENT ASSISTANCE TRANSPORT SERVICE officer supports patients to specialists appointments, surgery and hospital admissions in Adelaide; available Mon - Fri from 9 am - 3 pm daily. AFTER HOURS TRANSPORT: on call service Mon - Fri 4 pm-11 pm
